New Patient Registration

If you live within our practice area you are welcome to register with us and our reception staff will be happy to guide you through the procedure. Eligibility can be quickly confirmed from your address so please provide proof by way of a recent utility bill. We also require one form of photographic ID.

You will need to complete a registration form (GMS1) and a health questionnaire which will provide useful information whilst we wait for your medical records to arrive from your previous doctor. These forms can be obtained from reception or completed online via the above "Registering Online" tab.

Please note that it can take up to 72 hours to process your new patient application. If you need to be seen before this please call the Surgery on 664338 and we will refer you to our Duty Team for a callback.

Once you have completed your registration online you will need to book a new patient check and come into the Surgery to sign your registration forms to complete the registration process.
