Should you wish to make a complaint you are encouraged to try and resolve it straightaway with the individual involved. Where you are not able to resolve your issue in this way and wish to make a formal complaint you should do so, in writing, as soon as possible after the event. We are also able to provide you with a separate complaints form to register your complaint and this includes a third-party authority form to enable a complaint to be made by someone else. Please ask at reception for this or click on the links below. You can also provide this in your own format providing this covers all the necessary aspects for authorisation i.e. the name and address of the person you are complaining on behalf of and signed consent that they authorise you to act on their behalf.
